THE COVID-19 VACCINATION SCHEDULER IS

NOW AVAILABLE

The Oklahoma State Department of Health has developed a scheduler process for the COVID-19 vaccine.  COVID-19 vaccinations will be made available in phases to ensure that people that are most at risk are prioritized.  By completing the online questionnaire, you will receive an email when you are scheduled to receive the vaccine. 

 

We would like to encourage everyone that is 65 years old and older, to register for the COVID-19 vaccination.

Here is how you start the scheduling process:

  • Click here to access the Oklahoma State Department of Health website

  • Click on the "COVID-19 questionnaire" button to begin taking the seven page questionnaire

  • Once you have completed the questionnaire, you will be notified when your vaccination phase will begin to roll out
